This is a form that allows you add your name, address etc.. to a Five-Choice Application for printing. I use this so I don't get cramps from writing over and over. I use to use carbons but that was getting old. It was made Excel. Once you click on the link it would be a good idea to save to your hard disk for future use.
insert your name, address in the spots provided. Print it out on a blank piece of paper. (Ever printer prints it slightly different) After printing hold it up against a Actual Reservation Card up to a light to see if it is aligned correctly. Make adjustments as necessary. Once the adjustments are correct use an actual application to print.
